Children’s Targeted Case Management Program

Through its case management programs, Christopher Aaron  Counseling Center helps parents locate necessary services and supports within the community for their children. Case managers work with families of children diagnosed with mental illness, behavioral issues and other specialized needs, including developmental disabilities and mental retardation, to develop a plan tailored to the individual needs of the child. Case managers help parents:

  • Locate and coordinate resources and funding
  • Advocate for needed services on behalf of their children
  • Ensure that appropriate services are in place and that the children’s needs continue to be met

Tasks for the Consumer include:

Increase in natural supports
Increased understanding of self in environment
Improved coping skills
Integrating Life experience and promotion of resiliency
Increase in self-acceptance awareness and self esteem

Tasks for Providers include:

Understanding the nature of trauma
Not taking control over clients life
Seeing progress even when it is minimal in nature
Meeting the client where they are at and assisting them to move forward
Acknowledging that programs are made for people and not people for programs
Understanding Oppression and how it impacts clients at the Macro, Micro, and Mezzo levels and advocating for them at each stage.

A comprehensive assessment will be done that gives and overview of the clinical needs. All intervention up to and including assessment will be performed in a manner that attempts to not re-traumatize the client and builds upon their individual strengths.  All life domains will be assessed including family, living situation, legal, educational, social recreational, medical, and cultural/spiritual.  Treatment plans will be client centered and strengths based and will be focused upon emergent need first and triaged as needed.

Target Population:

Children and Adolescents with Developmental or Emotional needs
Families with threat of Out of Home placement for children
Children who have been victims of or witnesses to trauma
Families in need of increased understanding of child’s level of functioning
